Image MD uses information you choose to provide to respond to consultation requests and questions. The website does not currently provide a patient portal or medical intake form.

You may choose to contact Image MD by phone. Do not send medical records, detailed medical history, insurance information, Social Security numbers, payment-card information, or other sensitive health information through ordinary email.

How information is used

Contact information provided directly to Image MD is used to respond to requests, discuss scheduling, and provide information about services. It is not sold.

Website services

Hosting and other technical providers may process basic device, security, and request information needed to operate the website. Links to third-party services are governed by those services’ own policies.

Medical communications

This website is not for emergencies or confidential medical care. Call 911 for an emergency. Medical intake and treatment communications should use the secure system provided by Image MD.
